Acknowledgements and Release

In order to make volunteering a safe and enjoyable experience, there are a few things all volunteers agree to. Please read these carefully, and sign the form at the bottom acknowledging you agree to these standards.

I UNDERSTAND I AM SIGNING UP FOR A VOLUNTEER ROLE.

I acknowledge I am volunteering my time and efforts in service for Christ through the ministries of Elevation Church and that I will receive no monetary compensation or insurance benefits for my time or services performed.

I AM VOLUNTEERING AT MY OWN RISK.

I acknowledge and accept the risk of physical injury that could occur from my participation as a volunteer. Except for gross negligence on the part of Elevation Church, I accept personal financial responsibility for any bodily or personal injury sustained while I am volunteering. I understand Elevation Church does not provide insurance coverage for my role, and I will not hold Elevation Church or its staff responsible for injury, damage, loss, or accidents related to being a volunteer for Elevation Church.

Further, I release and agree to indemnify and hold harmless Elevation Church and its officers, directors, and representatives from any liability, injury, damage, loss, accidents, delay, or irregularity related to being a volunteer for Elevation Church.

YOU CAN TAKE MY PICTURE.

I grant to Elevation Church the right to make photo, video, and audio recordings of me as well as the perpetual, worldwide right to record, re-record, copy, edit, reproduce, publicly display, perform, and distribute the recordings in any way consistent with Elevation Church. I waive the right to inspect or approve the finished product wherein the recording appears.

WHAT I CREATE AT CHURCH BELONGS TO THE CHURCH.

I acknowledge that anything I create for Elevation Church belongs to Elevation Church. This includes works of every kind, physical or electronic. I agree that Elevation Church may make changes to the works without my approval.

I WON’T SHARE CONFIDENTIAL INFORMATION.

I acknowledge that while volunteering, I may have access to sensitive or personal information. I agree that I will not share this information without prior written consent of Elevation Church, both during and after my volunteer service.

I WILL HELP KEEP THE CHURCH A SAFE PLACE.

In order to ensure a safe environment for our guests, volunteers, and staff, I will not carry firearms or other dangerous weapons while serving as a volunteer at any Elevation campus or event.
